Here is the official answer!

As of our use of the Innovative system in June of 2003, patron can establish their own PINs using the My Account login screen of the OPAC (Online Public Access Catalog).
As of the Fall of 2008, patrons can use a "Forgot Your PIN" link on the My Account login screen to reset their PIN. To utilize this function, a patron must have a valid Email address in the patron record.

What about deleting someone's PIN so they can start all over again?
